Linked below is a reservation form for your upcoming event along with an instruction page. Please review sign and return along with your deposit to Andy's Taylor Rental. If paying by credit card please sign and return to the enclosed credit authorization form.

We accept Visa/ MC only.

20% down payment is required to confirm reservations.

• Orders with tents- total amount of tent rental (tents fees are non-refundable)

Down payments are due at the time the reservation is made.

All reservations must be confirmed and paid in full 3 days prior to the scheduled delivery date.

Any changes made to reservation counts less than 2 days out will be subject to a $25.00 change of order fee.
